An investigation into an accident that left a 10-year-old boy injured at Palace Playland last month found that the ride operator likely didn’t check a strap to make sure the boy was secured.

According to CBS 13 news, the State Fire Marshal’s Office has released its findings into the incident on July 16, in which a 10-year-old boy fell off the “Super Star” ride at the amusement park in Old Orchard Beach.

Witnesses said the ride continued moving after the boy fell to the ground and struck him.

An investigation found that the boy tried to get off the ride as it was starting because it appeared his safety bar wasn’t locked in.

The fire marshal’s office says the ride passed inspection when it opened earlier this summer.

The boy was treated at the hospital with non-life threatening injuries.
